The Kentucky Person-Centered Planning Network is an informal group of people who share a commitment to deepening their understanding about and utilizing person-centered planning tools and approaches to better the lives of people with disabilities in Kentucky. This website has been designed primarily as a resource to encourage people with disabilities, family members, community members and service providers to explore, learn about and utilize these tools rather than the traditional approach of searching for programs and services without consideration of the individuals’ gifts and capacities.

Person-Centered Planning has evolved and developed over the past twenty-five years into a rich body of work from which much has been learned. Initiatives of varying sizes have been present in Kentucky to both learn from and apply person-centered planning tools to assist people with disabilities to have real and good lives in communities of their choice.
